Death Certificates and Summaries
The state of Maryland requires all death certificates to be completed online.

Be sure to attest all areas so the death certificate is finalized. This is extremely important.
Be sure to place a working copy of the death certificate in the chart. Document that this is done in the Death Note [see below].
Death Note
If you are the physician pronouncing the patient, you are responsible for completing a Death Note (a.k.a. Death Summary). This should be done under a discharge summary template and titled “Death Note”. Follow the following format:
One liner summary about the patient’s history and reason for admission to the hospital.
Code Blue details. Or if DNR/CMO, the reason why house staff was called to bedside.
Date and time of death
Presumptive cause of death.
Time of notification of next of kin/surrogate [include name and contact information].
Name of Attending Physician notified.
Sign the note and send it to the attending physician to sign.
Place a working copy of the death certificate in the patient’s paper chart. Complete the “deceased person check list” with the bedside nurse.
