Check and clear your pool’s inbox.
Check Powerchart for patient results and send result letters (lab and radiology)
Check and respond to messages in Powerchart.
If you are calling the patient, write a brief note and forward it to the attending
Things to do each visit:
For each patient, update medication list, problem list, past medical/family/surgical/social history
Include a diagnosis code for all labs/referrals ordered
Give glucose logs, BP logs to patients
Clinical Depart Summary: follow up, med rec, sign/print.
Document a note when the patient is called and forward it to the attending
Check your EMR and Sinai email regularly to respond to clinic messages
Referrals:
Select “Ambulatory Referral”. Then select the service, click the binoculars, and find the physician.
Service by date should be one year from the date of the order.
Number of visits: 6.
Give this to the patient and ask the patient to call and make an appointment.
Forms:
All completed forms must be copied; originals can be provided to the patient and copies should be placed into “To be scanned” bin in each of the Pods
You are under no obligation to complete the forms on the same visit. Please tell your patients we reserve up to 5 - 7 business days for the paperwork to be completed and mailed to them
If you elect to complete the forms on the day of the patient's appointment, please do so if you have a break between patients or at the end of the clinic session for that day to avoid delays. Please ask patients to sit in the lobby and wait or return the next day to pick up forms at the front desk
How to order Durable medical equipment (DME)/medical supply prescriptions:

Nebulizers
Write “DME Prescription” in Cerner and document what is needed.
